Annual Chapter Report Form

Important
This report is the basis for which the annual Outstanding Chapter Awards are granted and listings in the SPS Information Handbook are compiled. In order for SPS Advisors to be considered for the Outstanding Chapter Advisor Award ($5,000), this report must be filled out in full and submitted by June 15.

If you would rather send a paper report via US mail or fax, please use our PDF or Microsoft Word© version, or use the forms found on pages 24-28 of your SPS Information Handbook.
